I don't think this one is even possible(aside from the triple miscs) because you can't equip the nose without the head showing up. I could be wrong, though.

So, I made my loadout request myself in GMod, and as it turns out the Medical Mystery clips pretty badly with the License to Maim.
As such, I'm looking for any suggestions for a second misc. item to go with the Medical Mystery and V. Tyrolean in Waterlogged Lab Coat. I'm going for a Noir-esque detective style loadout for this, and a new misc to replace the Maim would be appreciated.
For reference, this is how the hat and Mystery look together on their own:
