Just installed the mod and tried to change loadout but there is no option for it in options? Am I just being dumb or is it somewhere else? I've tried M.

If it's not version 1.8.3.1 then you're using an outdated tf2c.

I'll wager you just need to go through and manually install all the patches (they're not big though, most are around 4mb). If he/she gives up though I'll jump in and have a crack at it myself. For example if a screenshot has a barrel next to a unique piece of the map and that same barrel is in the current TF2, you can use it to try and scale a wall (e.g. this wall is 4 barrels high).
I don't know the dev team's current thoughts and a fix may already be in the works so I may be wasting my time. If it isn't, this doesn't have to be an immediate fix and can be put on the backburner since we've hardly even started with this mod but I'm just throwing this out there: [B]Giving Medic the shotgun is a bad idea.[/B] There are three reasons for this. One is that it's objectively better than the syringe gun in every regard. Syringes are incredibly hard to deal damage with, dealing decent damage when aimed well whereas the shotgun is powerful and easy to use, dealing enough damage even when aimed poorly. The second is that it's so much better it messes up the balance of non-combat classes(Engineer, Medic, Sniper, Spy) pretty badly. Not only does Medic have the best stats(25 more HP, 7% faster and HP regen) he now has the best self-defense tied with Engineer. And the third is that it's so good it puts Medic on-par with Scout. The odds of not only surviving but winning an encounter with a Scout or even a Demoman is absurdly higher than it ever should be whether the medic is alone or with a pocket. I'm not saying Medic is better than Scout but it's close enough that if the Scout misses a shot, he's got a good chance to die. I don't think that giving the Medic [I]a[/I] shotgun is inherently a bad idea, it's just that the current one is far too powerful. A lot of things could work. An idea I had some fun testing on my server was a TFC (Single-barrel) styled shotgun. Weak, long-ranged and fast-firing. It would make a good finisher but even worse than the syringe gun for actually taking down a class from full HP. /rant [B]TLDR[/B]: Medic needs his own shotgun because the default one is far too good compared to the syringe gun and puts him on-par with way more classes than he should be able to fight.
