I hope they make a 4th misc slot and change how action slot works, just make the action items equippable in 4th misc slot, but when equipped you can use them normally.

Nah, I think 4 lets me do literally everything I'd want to do. There's just a couple loadouts (heavy, engie) that I really want a 4th for, most of them I'm happy with 3.

Would be ok if we had that system Dota2 has where all the cosmetics are slapped into a single model, but I have a feeling TF2 would start feeling the lag if we did that.

It's all a matter of how well they can optimize the engine and the number of polygons in items.
Cosmetic items have gone UP in complexity and UP in number of polygons over time, but this has thus far been largely offset by engine improvements that give better performance on the same hardware.
If and when they add a fourth cosmetic slot, it will be when they feel that the engine will be able to handle the additional polygons that a fourth item would bring without noticeably dropping in performance.

I believe its more of an issues with the number of entities the engine has to keep track of. There's 32 players, each has one active(maybe 2 for demoknights) weapon, 3 cosmetics, and a canteen or whatever. That's 6 entities per player, up to 192 entities total. Dota went around this by combining all the items and player files into one entity. TF2 wasn't designed originally to have this much stuff.
Source can only render 2048 models at once and adding more cosmetic models onto player models added onto the prop models on a map would equate to a lot of fucking models.
Let's go back to Loadouts shall we?

The style on the veil.
Man, that made me realize how much I actually dislike face-covering cosmetics because it takes away the expressive faces of TF2 in every way.
I feel like the Scout there could use an (orange-painted?) Flapjack there to get closer to that sidekick feel, and I'd probably get a beard for the Sniper over the Veil.
Realized my Medic looks ugly, so I have a few ideas on how to make him look better. Thoughts?

The middle one if you want an authentic german soldier/captain look, the one on the right if you want more of a creative look.
I've always loved the Seeing Double and I think that it could benefit from some new loadout combination(s), rather than just The South of the Border set
