All signs have orange and white/black as their default if you don't use the glitch, so that might be it.

Well yeah, he's right behind the Heavy, so of course it will fail. For successful stabs you need to be in [I][B]front[/B][/I] of the target, or at most 70 degrees to one side. :v:
