No, it's already made. Like the rifles and stuff, it's a premade weapon, not one you make. Even the trailers on the prepare for the future site say it was "government made".

Only when aiming is otherwise impossible. I'll concede that it's genuinely useful for the 360 and PS3 because I've noticed parts in The Weapons video where the player decided to go to VATS because they couldn't settle the reticule on a target fast enough.
I can't say I'll be using it much. Effectively stopping everyone else so you can wipe the floor with them on demand seems cheap.

That's the thing about vats. If you miss or something, the enemy will have just as much time as you do to fire back. Theoretically you could take 20 shots at a head and not hit once, the enemy firing back the entire time. It's almost like turn based combat.

It's just that in the videos they always show them hitting. No one wants to see a video of a guy just shooting and missing over and over.
