Postal with some Mods can get pretty gruesome.
Manhunt 2 uncut is pretty gory and the game kind of sucks
Postal 2 with the Apocalypse Weekend and the AW7 mod is the goriest, most insanely violent game I've ever played. You get a scythe, you cut somebody in half, their intestines fall out. You cut off somebody's arms with a meat cleaver, they're still going to run around and scream. Not to mention that you can engulf the whole town in napalm, electrocute people until they piss their pants, get your dog to kill people, eat shit and plenty of other disgusting stuff.

There are zombies and other monsters, and you can chop off individual limbs from them. The atmosphere of each level is at least a bit creepy, but the insanity effects *really* take the cake.

But it's worth looking at for the detail of the combat/injury system. Name one other game where an archer can fire an arrow into an enemy, then a wrestler comes up and attacks the enemy, twisting the arrow in the wound to cause massive pain.
You know, wasn't the Quake series one of the first truly visually gory games?
Left 4 Dead 2 has tons of gore, I mean, there's an exploding limb every three seconds, but it happens too fast to make an impression. Looks cool, though. Gears of War 2 also. But none of those are exactly 'horror', so... Dead Space.

It's probably not the goriest or anything, but Space Station 13 can have a lot of blood and guts at times.
"The Suffering" and "The Suffering 2; Ties That Bind" are not only very gory and disgusting, but they are quite disturbing aswell. They are kinda like Silent Hill style psycholgical horrors. But more oriented towards action than puzzles.
GoW sucks and I'm sure there are some people who'll agree with me.
^ GOW, and GOW2 are great games first time round. I play them as if they were a blockbuster action sci-fi movie. But they have no replayability. They are painfully repetative.
L4D2 was banned in Australia for a reason.

Dwarf fortress... they take clothes from dead people, they dont care about dead animals scattered around, make stuff out of bones from dead enemies (wich are humans too sometimes) and all kinds of limb rip-offs and other stuff like that, if the game was better quality it would have been real horror.... imagine a guy being splattered against a wall in real life

Still i don't say military is perfect with that solved. One of my soldiers carries her baby around to everywhere, even in battle i think
