Or, the vortex could separate people in space and time, or "fragment" them, therefore, you can put in some encounters where slightly weakened Combine pop out of one of those, and then you wonder what the fuck. Plus, for extra realism, have some future wormhole encounters trigger based on how many grenades you threw in the past. It would make sense. Also, they're engineered to prevent the user from falling in themselves, preventing that sort of thing.
