I closed a door, turned around, turned back around and the door was open my reaction was "wtf?" So i ended up closing the door again and as soon as i turned around a fucking fishermen was standing 3 inches from me i jumped so far out of my chair.

Penumbra Black Plague and Overture are on my top list.
In this game you are defenseless, and that's what makes it so terrifying.
Trying to sneak around mutant dogs without getting caught and hearing them lurking around closer to your position is the most scariest fucking thing I've ever experienced in a game.
Then in Black Plague while hiding in a room those fucked up human/alien freaks spot you and you run for your life for the door and then when the other room loads you hear a loud banging sound coming from the door.

S.T.A.L.K.E.R Shadow of Chernobyl. The first blood-sucker I met....
The subways in Silent Hill 3, due to equal parts weird game mechanics, bad luck, and my own incompetence. I went down the side where you have to jump down to the tracks and check a door to proceed. The dogs show up and attack. I deal with them but don't know how to climb off the tracks (basically, which button to use). Heather gets hit by the train. Cue gruesome splattering sounds.
I learn how to climb, repeat, and still can't climb off the tracks. Apparently Heather won't climb up directly next to the railing for some weird reason (logically, I'd think the railing would make it easier by giving her something to hold on to, but...). Splattered on the tracks again.
I decide to try my luck on the other platform. Heather gets SHOVED on to the damned tracks by the resident ghost. Splattered again, thanks to still not realizing that there's apparently an invisible wall next to the railings.
I eventually learned the trick to getting her safely off the tracks, but that entire area still puts a huge knot in my stomach...

Intense chases are terrifying when you've made a tiny mistake and you have like half a second to correct yourself.

Penumbra and Silent hill comes to mind. When I was younger though, I was terrified of Tomb Raider.
I couldnt play long before I began to scream, when suddenly a huge bear or wolf would jump out of nowhere. Other notable frights were Hitler in Wolfenstein 3d and undeads in Thief 1.
Why the fuck there is a thread like this made by the same guy? Whatever. I was scared of L4D at the beggining so i played with god mode.
All of Penumbra both Overture and Black Plague I pooped myself.
