There are several reasons why that thing is scary. Lemme list 'em.
1. Like OutOfExile said, imagine seeing that at a very young age.
2. How could such a happy, cheerful game like Earthbound change into something so disturbing?
3. The music that plays. Listen to this.

4 .You never see Giygas until the very end of the game. The events that take place before the battle are a huge build up and when you see him, it's not really what you expect.
5. Most of the dialogue comes from a RAPE SCENE that the creator accidentally saw as a child.
6. Usually enemies in Earthbound don't take up very much of the screen, but Giygas...it doesn't just take up the whole screen, it he the screen.
7. "You cannot comprehend the true form of Giygas's attack!" He is basically invincible. The ONLY thing you can do is pray.
