When I played Penumbra over at a friend's house at 2 AM. We hear a sound coming from the garage and he just looks at me and goes "Mind going to check that out?". I told him that the only way I would enter that fucking garage under the circumstances were if he came along. We grabbed something to defend ourselves with and went out into the dark, we stopped in front of the garage doors and got our flashlights out. On the count of three we turned went in, turned on the flashlights and started swinging randomly. Out comes the neighbour's German Sheperd happy as can be and greets us. Never felt so dumb and terrified at the same time.
Was playing Half Life 2 for the first time, past midnight, on an insomnia binge. Remember that one spot where you go up to break the supply box and a fast zombie flies out of the water beneath it?
The Stalker games can build up a scary atmosphere. For example, running around at night in a storm, health depleting and only 10 bullets left. You never actually get attacked but you (or maybe just me) still shit bricks knowing what COULD happen.
In twisted metal black, where I saw Sweet Tooth slice Calypso's throat with a very small knife and that scary music played. I was like 10 or less when I played that. Scarred me for life. :/

When you are walking in that room with some frozen statue in it and you turn around and it's RIGHT IN YOUR FUCKING FACE.
I jumped up in fright, whacked my finger hard on the table as I did so and broke it.
I still have nightmares about STALKER Shadow of Chernobyl.
The part by the scientist bunker where you have to go inside this huge building complex, and shut off the brain machine. (I feel bad for not remembering the name D:)
I still recall those horrible eyes randomly roaming the place.. Then suddenly spotting me, letting out that horrible, bone chilling roar.. I still hate it to this day..
For startling I can sum it up in one sound: "SSSSSssssssss"
For actual scary I'd probably have to go with Silent Hill or Silent Hill 2. For the first time ever I really [b]didn't[/b] want to find out what was causing that noise.

in the first Dino Crisis, there was a keycard in a hallway late in the game. I try to grab it, but then a cutscene plays. something's in the room. Regina aims her gun at places, but it seems like it's gone... then a MOTHERFUCKING RAPTOR COMES RIGHT OUT OF THE FLOOR!!!
also, in the same game, those big scary black dinos with the huge claws. I think they were supposed to be Therezinosaurs, back before it was discovered that they were herbivores. I hated those fuckers.

The first game I shat my pants was when I was playing Painkiller. Why?

I wont cover this in spoilers, no one seems to have played this game anyway.
The back story of one part, an old woman in a swamp has died, and kidnapped a little real girl from a bunch of rednecks. You have to kill her, and save the girl. Trouble is, her sons and daughters have died, and turned into ghosts too. her daughters turn into said teddy bears i posted above, and her sons into little boys, who can turn into a mini tornado.
But one of her sons was eaten by a crocodile, and you have to beat him as a boss. He shows up, in an anti climax, as a typical crocodile. But the crocodile soon regurgitates a towering, blood covered, pulsing giant. It was disgusting, and to beat him, you had to make him swing the crocodile at you, destroting 3 shacks. When they were gone, the crocodile was fed up, and swallows him back up.
Ugh. Not scary now, but when i was a kid it horrified me.

I was just playing GTA4 a few minutes ago.
I had cleared out a few rooms of people when I rushed a little to catch up with the guy running away.
Unfortunately, I went the wrong way, so I had to travel back through the rooms. I said to myself, "Now, I probably missed a guy or two since I rushed through."
Sure enough, I'm running through and I go to turn a corner and there's a guy holding a pistol at me, needless to say he got gunned down when I had my moment of "HOLY CRAP OH GOD DIE!"
I'd either say, the time in FEAR 2 when i was in the first level and was in that low lit pool hotel area with the red lighting, i was just wandering around, and when i turned around, there was kid alma, just looking up at me. Fucking spazzed the shit out and jumped backwards IRL going "FUCKIN BITCH".
Or the time in Condemned when the mannequins kept following me around and i ended up smacking each one i came across.

Resident Evil was pretty scary for me back when it first came out. Also, venturing into the Kithicor Forest zone at night in Everquest when I was level 6 or something back when EQ was new.

Yeah that whole part was scary as hell.
They better have a similar thing in Condemned 3. Which they are making, because the developers were searching for an animator, in particular one specialized in martial art combat (so that pretty much says it) on their website.

I remember in Condemned 2 I saw a shadowy figure just standing there, I decided to be a man and charge at him with the melee I had, I wacked it several times before I realized it was a mannequin. I didn't have to look behind me to load before that.
