the reapers make-shift one. it's not a queen per se, just referred to as one, and you can not make a pact wih it
there are some decisions they had to throw out the window, so they do not have to make 2 different games

The only thing that carried over for my game was the facecode and even then I didn't have a facecode.

It's not two different games.
I'm not as mad at the ending as I am with how no matter what choices we made the end result was the same.
Same thing with the endings even. Same all the way to the very cutscenes.
They advertised that our choices would affect our game when all they did was change the codex or some comic book plot.

She should show up after Palaven and before [sp]the Cerberus attack.[/sp] She's shown up on both of my playthroughs so I can't really help, just make sure you aren't missing her.
