1. Well, isn't that making the game harder? It's hard(no pun intended) to make a game more difficult than to raise its difficulty standards. What more do you want out of the game?
2. Adaptive difficulty is when the game gauges the player's skill and sets the difficulty of the game based on those "measurements". Take Madden 09 for example, your "Madden IQ" was a system that scored how well you lpayed against the AI. If you beat the other team easily, the game kicks it up a notch. If the player just can't win, it eases up a bit.
3. Why such a hard outlook in videogame developers? If they weren't doing there job right, then they'd be out of a job. I don't know how you find hard difficulties to be "easy", maybe you're just really good at games, but I'm pretty sure from my standpoint that on most games I play, the hard setting is quite hard.
Try some roguelikes and download mods if you are looking for hard stuff.
Here are some challenges I will assign you:
Win a game on Deity in Civilization.
Build a fortress and last 10 years in Dwarf Fortress.
Beat I Want To Be The Guy.
Beat I Want To Be The Fangame.
BEAT A KOREAN AT STARCRAFT!
You need to play online multiplayer.
