I was playing on Nerdy's server, and after looting this guy's cave/mine of all its precious minerals, I came upon a offshoot cave that was only accessable via a 20-30 block straight shaft. I climbed up this shaft, and was greeted with a massive network of unexplored caves. As I went through them, I kept finding shitloads of iron as usual. When I finally decided it was enough, I was lost. Completely. I kept finding offshoots and more goddamn offshoots, that seemed to go on endlessly.
FINALLY after about 10 minutes of searching I find a tiny little 2x1 hole that leads up through a shitload of sand. When I get out, I find myself in a barely-subsurface cavern I had been in a while ago. The problem was, this place was about a 20 minute WALK from that guys mine entrance. Later, I glimpsed through a unloaded chunk. It was a fucking city down there. Caves as far as the eye could see, and I could briefly notice some torches dotted around.

I gathered up 56 blocks of iron ore. No doubt if I had continued exploring I would have found, much, MUCH more.
Let this serve as a notice that Nerdy's server is home to the biggest cave system I've seen since early, early alpha.
