Hybrid akali (gunblade + trinity) is pretty ok
only with AP akali you have way more build options.
So people only build hybrid when they have too much AP on their team.
And they have "Cho, Oriana, Lulu" and on top of that Akali and Ezreal have pretty decent ammounts of AP damage aswell.

Flash + Exhaust is used by supports, + Ignite is used by many mages and AD carries, although I feel Heal works better in bot lane.

Champions like ezreal and tristana have blinks and can force fights when they have heal and ignite to auto-win the fight. Also works on graves because of his tankiness even though his dash is pretty short range, but it still goes over some walls.

It's more of a champion specific thing, champions like karthus use flash + exhaust in mid whereas most other mids will use flash + ignite

In general you want to have ignite for pure killing potential and exhaust if you feel like you already do enough damage but need the enemy not to blow you up or escape whilst you dish out that damage

If you don't want to pick ignite, having a double exhaust can be a way better defensive option then the heal depending on team layout.

Heal is for pussies and sparkly vampire master baiters, ignite provides so much more for an ad carry imo
