It seems I am not alone. it could be the vehicle approaching maximum dynamic pressure... or simply the engine crapping out. Is there a debug mode where you can see the forces acting upon the craft? at least in a numerical form
If that's happening, you should use more support struts and make sure nothing's wobbling around. Definitely be sure to tie any central columns to outer stacks.
Try to throttle down from 100% to around 60-70% for a few seconds and then see what happens.
Behold, tales from Minmus!
Jeb, Mitger and Corwell flew to minmus, their rocket landed on one side, capsule fell off. Luckily, the crew tank was still attached to the fuel tank and engine! Corwell proudly volunteered, and gave it the bees knees, the whole thing explodes, nearly kills Jeb and Mitger standing besides the craft (Should have moved them), and throws the tank half a kilometer away from Jeb and Mit. Engine flies off to god knows where. Luckily, everyone survived, and I shall now attempt a rescue mission!

Around 40, maybe 50 seconds from Melbourne, but my first download dropped the connection after 500kb for some reason. Second and third downloads were flawless.
Only wishes: sideways ladders, ability to have more crew pods.
Also if you're landing by parachute, you can eva your kerbals and have them ride down on the ladders. I've tested it.

You can rotate ladders.
Also, I had the brilliant idea to bring jeb bill and bob over to mission control in a buggy, then eva them so they could sit there and watch the pretty launches.
I start the next mission, and it still had them in the crew rotation :v

Generally if my first couple of launches don't work out I go back to the drawing board
Also after having a craft violently shake itself apart outside of Kerbin's atmosphere, I don't think it's dynamic pressure.
We're probably just not used to the limits of the new parts compared to the power of the engines.
I think that the planet needs some cities dotting it and a road system maybe so it doesn't look so barren. (Also so we have some stuff to crash into.)

SAS tries to stop changes in angle, although if you only are using your pod's SAS and/or one SAS module all this means is it'll slowly stop you spinning (useful in space).
A single ASAS module will actually use your controls to keep your ship pointed at whatever attitude it was at. I think you want an ASAS module.
