Even slight differences in velocity and distances can cause a miss in orbit. Crashing two rockets into each-other requires extraordinary precision, with nearly perfectly opposing (or aligned) inclinations. Even if the difference is only a few decimal places, that can still make them miss each-other. Are you using MechJeb?

The planet rotates, so you're not starting from the same place every time.
That makes the orbital planes different.

It's either your planes and altitudes are still even SLIGHTLY off (just calculated, at a ~100km orbit over Kerbin, 1° of difference in two orbits could cause a maximum distance of 12.2km between two craft.) At two orbits exactly equal in terms of altitudes (impossible by itself) you'd need a difference in inclination of less than 0.00025° in order for two craft to hit without any guidance, assuming the crafts are about 3m at their widest point.
OR, if you're certain MechJeb has done it's magical semi-imprecise magic and the orbits are literally exactly equal as you are being told by the information displays, could it be the poles are fucking with shit? I remember in previous versions of KSP the poles were buggy, not sure if that's been fixed? Or maybe it doesn't have anything to do with it all, not sure.

It works nearly perfectly every time for my smaller/medium sized rockets, but for my larger payloads I've been relying on good old manual control for them to reach their objectives.

Anyone have a recommendation for mods that add command modules and pieces for space stations? I have one that adds airlocks, but a major problem is that they don't have ladders attached. Kerbals just jump out of the pod which is obviously problematic in the vacuum of space traveling at hundreds of meters per second.
I just want something that looks more space station'ey, and less rockety for my KSS(Kerbin Space Station) command module.
I had a go at orbital rendezvous without any mods.
I got pretty close:

Space and N are quite close together. Also apparently there's a thing called quick save and it's a good idea to use it.

KOSMOS? Kosmos has at least 30+ parts of all kinds to use on space stations. Plus, the KOSMOS VA command module looks the part IMO.. just add the 2m housing pods or whatever from that addon.
