The game is rather old
I was actually very impressed with L4D2's gore, pretty much wherever you shoot them will leave a big gaping hole. Like if you shoot a zombie in the upper chest with something like a Desert Eagle, then it will blow their chest and throat out, revealing detailed bones and organs, and of course the exposed windpipe of the zombie :)
The blood squirting effects are pretty good too. Infact the gore is the only reason I'd buy L4D2, I'm fucked up I know.

That's the engine at work. Ingame theres many sprites and decals to make it look more gritty. For example shoot someone in the jaw, their jaw will break off and a stream of blood will squirt out, take out a section of the head and their brain will be visible and gushing blood. It's really nice.

To be honest, It looks like it's in the same range as the "Terrorist Takedown 2" and "Armed Forces Corp" style shovelware that gets spawned from obscure companies in former Soviet bloc states...

The game is over 7 years old, cut it some slack. No other game has had that level of gore since, so I think it deserves props for that.
Still waitin' on my DMM gore.

In terms of quality, Red Orchestra. Game has some real brutal yet realistic gore, especially if a grenade lands near you. Soldier of Fortune 2 has some rather brutal yet realistic gore also, shooting someone in the head with a high caliber sniper rifle mutilates their head; The shotgun is exaggerated though. Pretty damn satisfying to blow someone away limb by limb however.
In terms of quantity, HL2 SMOD.
