Fallout 3 is pretty gory, with eyes and such things. However it doesn't come that far in the contest of ultimate gore.
Manhunt 2 AO edition probably wins.

Actually, I'm not sure. There are probably loads of games that were banned for release that would qualify to be the most violent.

I don't know... Condemned is kinda brutal, perhaps not the goriest though.
In Soldier of Fortune 2, it's so satisfying to shoot off little chunks of their body with a shotgun. Imagine the GHOUL gore system in modern games now, or an upgraded version.

Yeah, Condemned is pretty brutal.
I remember shooting a guy with the tazer, and while he was in shock I grab the handgun from his hand and fire 3 bullets in his chest.
He just made a weird noise and collapsed to the ground.
It doesn't sound very violent, but it was quite impressive to see.
In terms of gore, SOF 2, hands down! You have incredible detail in wounds and damage. lets see, we have
-Removable Limbs (You literaly rip someones face off bit by bit, shoot off hands, feet, upper arms, lower arms, or quater a torso)
-Damage decals for bullet holes (Exremely detailed g.e. Burn marks from close shotgun hits, soaked clothing, splashes)
-Blood spurting from artery hits
-Exploding Bodies
-Mutilate corpses for fun!
An then there is the Punisher game. Many games tried to pull off the quick kill/interactive torture thing (manhunt 2, Condemned 2, Wolverine) but it never worked
as sadisfying as in this game:
-Removable limbs (only arms, legs, head)
-Damage Decals for bullet holes (wich are unlimited, you can turn a body into swiss cheese)
-Interogation scenes (This is where the fun starts: Interactive torture scenes, Use Circular saws, forklifts, Elevator shafts, Woodchippers, Meat Grinders or even an innocent looking well to fuck up your enemies)
-Quick Kills (Extremely stylish killing animation, based on your position and the weapon youre holding. Theres a ton of them, I always discover new ones)

It's awesome beating the crap out of a phsyco killer with a pipe. If I recall correctly you also get some ways to end them sometimes, like break their necks, kick them in their face while holdinng them and so.
