and why would you initiate with pool? It has a long cooldown so it can't be spammed in a team fight, and since you maxed it out, your q will be piss poor and have a long cooldown too.

Pool is a one point wonder. You are supposed to get a bit of CDR and max Q to spam it a billion times in one teamfight.

Tides of Blood will deal 327
This is on PERFECT conditions with enemies averaging at 80 MR

If you're trying to kill someone in your lane, you should
pop your ult
Tranfusion, drop into your pool (to stop them from getting away), Tranfusion again (if you maxed it out first), Tides of Blood if you can and this should already kill them, or your ult should.
Extra points if you took ignite with you and ignited them right after you ult.

Scrap that Spirit Visage. It is nice but not really worth it
Get Haunting Guise after boots and Rylai's after that. Rylai's is one of the best items on Vlad imo

Scrap the Visage and Guise and the Doran's. Start with an Amp. Tome. Feeling good about the game? Make it a Soulstealer. Not so much? Make it a Rylia's. If you do get the Soulstealer, make a Rylia's next. Deathcap next. Void Staff after that. If the game isn't over by now, Hourglass.
E: And never, [b]ever[/b] get Warmogs. Any Vlad who gets Warmogs is a scrub and bad.
