I only get a philosopher's stone at the beginning and after that i just build AD and cdr. If the other team has a few fed champs then i'll get one defensive item.

Yes, this happens to me a lot in ranked.
Whenever I go on losing streaks matchmaking feels sorry for me and puts me against a really bad team and I proceed to stomp them
Then it's back to losing
I'm currently on a 6 lose streak.

My core build is philosopher's stone, cdr boots, manamune. After that it's your choice of AD items. My final build usually ends up being cdr boots, infinity edge, stark's fervor, phantom dancer, black cleaver and shurelya's reverie. Also, it's a good idea to build a brutalizer early game and then sell it for your 6th item later.
