On singleplayer games I always use default names.
On multiplayer games I change it to something that fits the character, but is unique. If it's fantasy, I get the name from a D&D name generator.
Yeah, it's totally fine to have the names "NAME" and "Player Name Here" through all games.

I used to rename all of my pokemon Dick when I was younger. I found it hilarious until I couldn't remember which one was which.
I always gave Garry from Pokemon the name FAGGOT or ASSHAT or something.
