Looks like a new style for the difficulty icons.
I like the style of what I've seen for the new menus but I don't really like the layout of a lot of things.

That might be true, but they went out of their way to create something new that is similar to Narrows, as you can see it's not a direct copy/paste of the MP map from Halo 3. Now, this one has AI so I doubt we'd see it as a MP map, but we could pass through some Narrows-like areas in Halo 4's campaign.
To me it looks like it's running on Halo 3's multiplayer with the graphics toned down. I don't think they went out of their way to remake the map for proof of concept testing.

Not really. If you look at the extrusions you can see they're shaped differently (it's built out of multiple columns put together instead of a sloped elevation), and the illimunated parts are blue instead of yellow/white. The base is embedded into the rocks instead of being a big construction that pops out (and covers) the rocks, as seen here:

There are too much differences between Halo 3's Narrows and the map in there for them to be the same, even toned down, unless they'd use an Alpha version of Narrows, which I highly doubt.
One of the maps in the video looked like they were running it on Half Life, it's possible they just took an existing map and added/changed things, or make a quick mockup map that resembles an older map from a certain area.
I think I saw sentinels at the end of the video, but that shouldn't be surprising, Sentinels core forerunner machine.
