Are there any? I mean, with combat like in DDO. Point and click combat is bad. Preferably free, doesn't have to be graphical.

Age of Conan is mildly skill based.
You'll have to look up the dynamic shielding and attacking system on Youtube, it's pretty unique.
Planetside and Darkfall I believe.

I mean something like instead of clicking on someone, you have to rely on the player's own skill to fight and win. Not the character's skills, but how well the player plays a game.

I think Guild Wars is a prime example of a game that ISN'T skill based.

Well yeah, actually, GW PvP is very much based on your skills rather than how much you've been grinding like in some MMO's.
of course the fact that nearly everyone uses the same goddamn flavour of the month builds kinda sucks the fun out of it, but hey that makes it all the more satisfying when I beat them with my half-assed mesmer build I shat out in half an hour

Guild Wars is completely based around player skill. Max level, armor, and weapons are easily accessible, so you are forced to utilize skill.
And Guild Wards doesn't have a point-and-click combat system?

Which is more like strategy, figuring out which skills to equip your char with. It's cool that Guild Wars does away with (most of) the grind-fest, but in the end, it comes down to using "skills" and clicking the left mouse button repeatedly on enemies, watching numbers pop up above their heads. I'm so fucking over that.

You'll go fucking crazy with the amount of skill needed to even understand the first rung of the 2,000 rung ladder that is EVE Online.
If you want skill based mmos, try these games
City of Heroes/Villians
EVE Online
ACE online
Mabinogi (Not heroes, but anime theme version. I play with auto attack off, better than just letting the computer do it, although you wear down the sword faster, you land crits faster though.)
and a few I forgot (Will edit once I remeber)
If you want to play ACE online, is almost as crazy as EVE, but with only one plane/ship that you have, and if you like sci-fi stuff, this is the game for you. (Suggest that you go through the tutorial, or you will be guessing the controls while playing. Free to play.)
Pros: Nice graphics, action and skill based, many skills and weapons to obtain with all the plane types
Cons: Only one plane/ship you can have, four planes you can pilot (one per character), only 2 nations (once account is with one, your locked with that nation, untill deletion of all characters.)
Plane types are (called gears):
I-Gear (Fighter.Fast attacks and speed, low defense, need to upgrade that yourself by leveling.)
B-Gear (Bomber, and not ground bombing, heavy attacks.Now found out, has a ground bombing attack, makes missles curve down to ground.)
A-Gear (basicly a flying tank, that hovers on the ground. Prefect against both air and ground. Seperate ordinace from other gears.)
M-Gear (Support plane, like giving power boosts to you teammates type of support. Can also fight though, doesn't just sit back.)
Ace and Eve are okay, but I've played City of Heroes. You click on an enemy, run up to them, and attack, but you attack by pressing keys like 1, 2, 3 and so on.
EVE is probably your best bet, but be aware the learning curve is pretty steep. Thought not as bad as it used to be.

EVE Online is mostly based on numbers and knowing every single item, fitting, ship and pocket of space.
The simple rule is, the more people you have in your fleet with better ships, you win.

Great minds think alike... and by think alike, I mean I am cancelling your casts and replacing them with my own. Mesmers FTW
But yeah, I wouldn't exactly call it entirely skill based... much of your ability comes from your build selection. The rest is about using them right. Skill is required, but not in the same manner you're looking for. Try Planetside, Darkfall (take this one with a grain of salt) or Mortal Online (when it comes out). These games are primarly based on player skill. Planetside is even less reliant on in-game skills as your Battle Rank (BR) only affects how many certification points you have, and cert points don't make you a better fighter, only a more versatile one. What I mean is, whether you're BR 6 or BR 25, the main assault rifle still does the same damage, but the BR 25 guy has more cert points available so he can use more stuff than the BR 6. He just can't use it any better or worse than the BR 6. It's an interesting method of rewarding players.
I've heard World War 2 Online is much like this, but has a learning curve not unlike that of EVE. Be prepared to invest a lot of time trying to understand what the fuck is going on.
It isn't out yet, but you should check Blade & Soul.
Like some other people have said, I would give Guild Wars a go.
Probably would want to start with Nightfall first since it gets fun faster than the first but still gives you time to learn and get adapted to it unlike Factions.
if you want to totally skip the grind in GW, just buy the core pack and the PvP access pack, you can't play any of the campaigns but you get access to every pvp area, all classes, and 40 or so free unlocks for skills
