Having something that is not necessarily scary, but finding out that it has a freaky back-story or a hidden meaning.

You're a complete dumbass.
Oblivion actually managed to be pretty creepy sometimes. And the dunwich building in fallout 3 was even worse.
This, especially with all of the Lovecraft references in it, the hallucinations, etc. It takes a lot of self-convincing whenever I need to go back there for a bobblehead. :ohdear:
That one level from Thief 3.

i will never fall asleep after going to jail again
What's the story about this NPC?
Going around a corner and walking into something.

I actually read some Lovecraft after I finished the Dunwich Building. But I forgot to get the bobblehead, I was too absorbed in the atmosphere of it.
Crawling into vents in FPSs
definitely the regenerators and iron maidens from RE4 I couldn't go past the part where you first meet them for over like a year, then I finally found out that you could kill with the thermal scope.

Taken from her Unofficial Elder Scrolls Page
Myvryna Arano is a Dunmer agent who lives in the Waterfront District of the Imperial City. After you have been to jail she will travel throughout Cyrodiil to find you and deliver your invitation to join the Thieves Guild. However during the quest The Elven Maiden you find that she is a mole hired by Hieronymus Lex to infiltrate the ranks of the Thieves Guild in order to point Armand Christophe out as a Thieves Guild Doyen and to pin him for the Statuette stolen from Lady Llathasa Indarys' tomb in Cheydinhal. Sadly, this does not matter much to Lex and he arrests her, despite her loyalty. After The Elven Maiden you can visit her in the Imperial Prison. Oddly, she does not switch to prisoner clothes, and even more oddly, they let her keep her dagger in prison.

That Dunwich building in F3....... I couldn't even go in it without music playing...

I didn't even realize all of the effects were there... just the setup of the building and Fallout 3's ambiance were enough to make it terrifying. And that was before I had Charon to protect me.
The void in Hammer Editor kind of scares me...

Charon makes it worse! All those Ghouls, and suddenly here comes Charon the Ghoul WITH A SHOTGUN! Oh joy! Also applies with Fawkes for anywhere ever. Nothing ruins your day faster than a Super Mutant with a Laser Minigun, especially once you realize you just killed your Super Mutant with a Laser Minigun.

Put Charon in power armour. I give him the T-51b.

But I fucking love to see Charon without a helmet on. It lets me know I've got a zombie for a friend.
Crawmerax the Invincible, from borderlands
IT'S NOT POSSIBLEEEE

Yeah. I'd be lying if I didn't say that this level got me in to Lovecraft, and now I love his works. I've read a number of them and absolutely love them.
Unfortunately, I'm too much of a coward to play the Call of Cthulhu game. :ohdear:

The game is meh. You don't have a gun for the first big chunk of it, and at first it's awesome, but it's not really non-gun friendly. There are times when you have to use stealth in a game that has a very shoddy stealth mechanic. And then you get guns and it becomes Call of Cthulhu: Modern Warfare

Do the guns have unlimited ammo? I remember once I watched a guy playing it live on justin.tv or wherever and it seemed like he would just throw millions of bullets at everything that moved. I also pointed out that the saves looked like penises and ruined the atmosphere for everyone in the room.

You don't get infinite ammo but for the first third you don't have guns but you collect ammunition that whole time. So you will have plenty of bullets.
That scene in HL2:E2 where you first see the Hunters. Scared the crap outta me when it looked at me through the window.
Now that I think about it, any Hunter ambush scene in that game was scary.

she interrupts your sleeping to give you a note to join the thieves' guild
scared the shit out of me the first time i played

Or worse, she'll creep up on you as your sneaking through a pitch black dungeon. Those red eyes... Almost gave me a heart attack.
When it comes to horror games, I know what I like and that's dark corners. Condemned, stalker, etc.
being out of the map in every fps game, its like god is hunting you down
example:
borderlands:turrets shoot you down
most:blows you up
Traps.. everything from mgs3 to fallout3/oblivion like mines and spikes.
I always shits a brick when you hear the trigger to a trap.
I dunno, usually when there's a timer, and when you run out of time, loads of freaky ass shit comes out of doors and kills you. That's freaky because if you get lost you're like 'Holy fuck.'
