I'd have to say fallout 3. Combat shotgun with a few mods I d/ld has laser sights and has a great firing rate.

Unreal Tournament 2004. Flak Cannon. Although it's more of a shotgun/grenade launcher combo, it can be devastating if you can master it.
I've gotten good enough that I can lob a grenade directly into someone from a dozen meters away. Unless they have shields, that's almost a guaranteed kill.

That's the only gun I ever use in UT2k4.

Doom 3 too, Id didn't really change much of the damage the weapons do. Plus you find shells every corner you take.

The "1897 trench gun" in CoD2 multiplayer was insane. It instagibbed everyone inside a 60 degree angle from the shooter. It was banned from a lot of servers because it was so powerful. Definately the most powerful shotgun I have ever used.
The shotgun in Gears of War wasn't that overpowered, it was just because almost every other weapon sucked ass online, especially the Lancer and Hammerburst. If you faced off against other players in environments with cover, you had to use it. GoW 2 is way more balanced, and I think the third will be ace.
The most realistic shotguns I have ever used in a game were the ones from STALKER and Crysis, because they could all change ammunition to be more accurate at range (Which everyone that have fired a 12 gauge shotgun knows)

Another vote for Doom, and Serious Sam.
And ironically, in both games, I find the double-barreled shotgun to be inferior to the single.
