They should expand to Canada. I'd like to see snow combat in Fallout 3 style. They did it well with Operation Anchorage.
Using my ability to see predictable plots i'd say: Tandi died, A Evil guy (or desperate one) is leading NCR, Things are pretty much fucked up in NCR and they're going to attack settlements nearby to gather supplies and take control over that area. I think they are re-using the Fallout: Van Buren plot thing, So and NCR is proably at war with the BOS
Looks a lot more like the first two Fallouts, with big cities and such.

And none of the things you listed are "plot holes", just plot points.

Dams aren't targets for a nuclear attack. That's just over kill. Hoover Dam exists and from my understand it is still functional give or take some elbow grease. Infact NCR and The Mid-Western Brotherhood of Steel(?) Could be the original BOS, or one of the many raider groups were fighting for control of it at one point. At the bottom of the dam there's a slummy area, and the dam's rim is home to all the rich folk and military. The Dam became part of the New California Republic some time after 2242. The town has lost contact with the central government of the NCR in Shady Sands, and Governor Joseph Dodge, the leader of Hoover Dam assumed that it was destroyed. He's determined to hold the NCR together, and declared himself the president of NCR (or what is left of it). Unfortunately, Dodge is a weak leader, and he fights an ongoing war with the Brotherhood of Steel.
The ex-lead for Black Isle Studios is the lead for Obsidian, the dude that was behind Van Buren is the project lead, and the guy who wrote the Fallout Bible is a senior designer. Needless to say, the story is going to be very good.
