Not sure if this is notable, but I've successfully united half of Ireland into the Duchy, with my Current Character holding 3 titles. One hint: Watch out for Scotland and England. Sometimes they suddenly think it's a good idea to pick up another war.
If you unite Ireland, it might be worth looking into uniting the entirety of the British Isles Scotland's less dangerous than England but really just pick on whichever one's in worse condition and only when it's advantageous for you to do so. Aniother potential conquest is Iceland, not that there's anything really on Iceland.
Sword of Islam has been released on Crusader Kings 2. I'm waiting on the Steam Release.

Never seen that happen before. Also in my experience Scotland always likes to take a big chunk of Northern Ireland for themselves, but it doesn't look like they'll be doing that anytime soon considering The Isles. Your biggest threat is England. They like to invade Southern Ireland, and they bring in armies of around 20k, so there's not much you can do about it.
I never got that there are independent duchies and counties. I'd stylize myself as a King/prince if I'm independent.

Declaring yourself to be higher up in the feudal structure would be pretty dangerous, and most likely seen as you getting ideas above your station, assuming that other Lords even take your claim seriously (which they probably won't). At least I think that's the case.

He won and now she's the Queen of England.

My lustful King was having an affair with one of my unmarried Duchesses, I expected my third bastard but instead got a breakup event, such a shame. Then I noticed she had married someone, her nephew. Ew

If you were an independent count, you ruled a County which contained a certain number of Baronies and Estates. A king was someone who ruled several Duchies and had Dukes who swore fealty to him. It's more a matter of semantics. An independent Duke was really the same as a King, but he didn't have the fealty of a bunch of other Dukes.
Most of them did style themselves as Princes and Petty Kings, Navarre for example. If you didn't have much land though and a big powerful neighbour breathing down your neck, it wasn't a good idea to puff yourself up too much. Same reason so few countries declared themselves Empires, it brings a lot of attention and plenty of wrath.

For me Denmark, Norway, Sweden, all of Italy south of Rome and all of Eastern Europe is completely missing its coat of arms, while France is using what looks like the Coat of Arms of Corsica.

Maybe it happens to people who did not download the update prior to installing the expansion? In that case you should verify the integrity of the game cache. (advanced application options @ steam)
I guess my nation is too famous for our old coat of arms. The old coat of arms changed to something new. :c
