Personally it was Gex 3: Deep Cover Gex, I was young at the time and one specific level scared the shit out of me.
I'm talking, of course, about the one where you're a parody of Sherlock Holmes, the stuffed grizzlies that came to life and chased you scared the shit out of me.
I was more scared of that creepy lightning noise when you looked at the Dracugex poster. Creepy shit.

That game was fun. And I liked killing those freakish monstrosities.

Fallout 3 scared me the first time I played it, that fucking vault 82 was creepy before I discovered the light function of the pipboy.
Fatal frame.
Fallout 3 ghouls in the metro system scared the crap outta me.
That and when I first played the HL2 demo a long time ago, it was all like "Nice little combine controlled city" Next its all "Nice little zombie covered town with a fat guy that has a shotgun". I look back on those days as I string zombies up with the toolgun.
Duke Nukem 3D :ohdear:
Toy Story 2, damn that robot in the first level gave me BAD nightmares...
there was a level in enter the matrix with a closet and a mannequin in the background
i thought the mannequin would come to life and attack me or something so i shot it in the face a bunch and after that ran away and never did that level again :saddowns:
Hmm I played that game. I was scared of this zombie level or something can't remember was only like 6
Toy Story 2 for the N64. I just recently played it after a nostalgia attack from watching Toy Story 3 (Great movie by the way) and I had this dream that Zerg had locked me in a cell aboard a large plane and was flying me somewhere to be executed. It was one of those "No matter what you do you can't do anything" dreams.
It's not a scary game (Except for that Jack Hammer), but it was a really dreadful dream.
It was on the N64? I could have sworn I remember playing it on my PS1. Anyway, that game was aweosme
Dead Rising, I just couldn't get past that scene when you first get into night time. Those blood red eyes, oh god the blood red eyes!
Holy shit. Now that I recall, the FUCKING GAME BOY CAMERA used to creep me the fuck out when I was little.
