These. Wolfos in general were frightening, but they didn't give me the paranoia of wondering when the next one is going to pop out of the snow, let out the blood-curdling howl and scare the shit out of me. I remember for the longest time, I never wanted to touch Majora's mask after white wolfos. Eventually I manned up and beat Snowhead.
The n64 zelda games were good at having scary enemies that made me stay away. Dead hands, re-deads, those skeletons that would pop out of the ground before you pulled out the master sword, those skeleton fish, Bongo Bongo... to name a few.
But then you play it when you're older and more desensitized to scary shit, and then you wonder why you were afraid in the first place. And then re-deads happen and you remember all over again.

S.T.A.L.K.E.R. Call of Pripyat. That cave with the Controller in it. You know, the one by the burnt farmstead. That voice freaked me out. I'm not kidding.

Metro 2033, the station where[sp] the mutants broke in and ate EVERYONE, little rat buggers popping out everywhere and children screaming...[/sp] nightmare fuel.
Clive bakers undying and Call of Cathulu
Return to Castle Wolfenstein(I hate that level when you must escape from some catacombs full with ugly demons)
The aliens in HL1 didn't scare me. The water didn't. Xen didn't. The tentacle thingy didn't.
The opening of the game where alarms are blaring and the facility is exploding scared the life out of me when I was young. No other parts of the game scared me though.

Fallout, but only because of the creepy faces whenever you talked to people.
Unreal 2, but I was sick at the time so that might have contributed.
They need to have a game, where if you pause, a scary enemy can still go after you.
it's called Dead Space. :eng101:
Alone in the dark as a 5 year old on my friends new play station.
Traitors Gate, I don't really know why, maybe because you were so... alone!

Portal.
I mean, just a few days ago, I had a really bad nightmare that had to do with GLaDOS putting me in eternal cryostasis, but after that, I don't remember anything but PURE HORROR AND TERROR. I was literally afraid to think about that game for the day.
Dead Space. Fighting that big necromorph that was pratically indestuctible gave some minor nightmares.

The first time I fought a feral ghoul in fallout 3. It came from behind. But I wasn't scared of them anymore when I figured out they are weak as shit.
There was this one game I forget the name of, but it centered around fighting ghosts, zombies, and these weird spider-head things, and it was in a 3D isometric viewpoint. That game gave me the creeps imo.
The end of Penumbra: Overture. [sp]I backed into the wall when the hallway lights turned off and didn't move for ages.[/sp]
I never had a nightmare in my life.Only sad dreams and no game ever caused me to be scared after playing, only when playing it.
