Considering getting back into WoW. I quit playing a few months after BC came out and only got to about level 40. I really enjoyed the experience but found that it was taking up too much of my time and I was turning down nights out etc to play it. I'm now pretty confident I have enough time to pour into it - is it similar to before? Has it changed much? Is the free trial any good?
1-60 has been reworked, there's all the expansion pack content that you haven't seen yet too. The trial is the same as it always has been, and it's a lot easier to level these days than it was in vanilla/BC so it probably won't consume as much time.

Is there any mods that are pretty much required? When I played all I seem to remember using was some quest helper thing. I'm so out of tune!
Quest helper is built in.
Yeah quest helper is basically built in now, there is an arrow and everything and most of the time you don't even need to read the quest text. A big part of levelling now is doing dungeons with the dungeon finder too, so there's addons that can help you out with that such as Deadly Boss Mods, Omen and Recount but they definitely aren't crucial until 85.

but have a look around curse and other addon websites for some preferential things, for example before I used a fully custom UI I hated playing the game without Bagnon, and you can find other tweaks like that around the net

The price is justified due to the amount of hours you would theoretically put into the game.
also you don't have to pay for all of the expansions at the same time, and considering games like MW3 cost $60 and have 4 hours of content I think it's worth it
I've been playing since 2007 and haven't regretted spending a penny

I really don't think people earn enough money from old content to justify a nerf. I know I run a few old raids each week and usually get around 250-1k gold or so, nothing more than what I would make from farming elementium or something for the same amount of time.
No fun allowed I guess.
Does anybody here think WoW will keep enough subscribers to continue with expansions over the next 2-3 years?

I can see why, I used to run Ulduar with a friend of mine and in the process of 2 manning it we would make about 1.5k.
So I guess in Pandaria that sort of content will be soloable, meaning 3k off of just one raid run every week.

MoP hasn't caught my imagination to be honest, but they may pick up more users in Asia to make up for the ones they could potentially lose in the EU and US.
I think it's Asia that's been declining actually, in the last quarter 1.1m subs dropped and I'm fairly sure the majority of them were Asian subscribers (don't quote me on this). Though I'm sure the fact that it's summer and the build up to a new expansion is likely also a big part of it.
The reason why Blizzard releases new patches is to maintain the product life-cycle of WoW.
When WoW was first released, it went through introductory phase, growth phase and maturity. Since level 60 was the limit and people "finished" the game, they grew bored and stopped. WoW came into decline. That's why when Blizzard noticed the maturity phase (the level of income generated from WoW coming to a certain level), they start to plan and brainstorm a way to recycle the product life-cycle of WoW. They add new content, new animations, new lore, etc and therefore WoW gets placed in the growth phase again since more people are introduced into WoW Vanilla and existing and new members also buy WoW Burning Crusade. They did the same strategy for Cataclysm and now Mists. WoW is a money maker for Blizzard in terms of profit and investing into coming up with other products such as Starcraft 2 and Diablo 3. The release date isn't just some date they choose by random. There's always marketing strategies. They predict the right moment when WoW has begun to reach end of maturity/decline phase and release it to recycle the product life-cycle.
Sorry for the essay, just wanted to give my 2 cents of marketing knowledge.
