they're just silhouettes, they're not hollows.
i like the idea that the entire game world in dark souls is a sort of afterlife or hell, seeing as how they mention the living world a few times

They could be hollows too.
who cares hollows weren't around until the curse of the undead which didn't happen until flames began to fade, the silhouettes rising out of the first flame were the first of humanity
I thought the game explains though that the Hollows only appeared after the dragons were dead and the Age of Fire started, and that it even states that Nito was the first undead.
Plus, the way the game phrases it

Makes it sound like the Hollows are directly linked to the Age of Fire. Regardless, it's fairly vague and could swing either way.
Dark Souls lore is somewhat confusing when trying to understand the big picture, honestly what I really care about is how sad it is to see Sif limping and trying to kill you and protect his master's grave. :(

next two times I was smashed into holes
All I did for Centipede was sit in that big corner and smack his legs
Also some guy dressed as the grim reaper kept invading me and we killed eachother at the same time twice

Bed of Chaos kept knocking me into the holes that open up. It's a very short fight though, and if you get past the arms, you're golden.
Is it ever revealed why O and S attack you, it seems odd - are they supposed to test any undead to see if they are worthy of an audience with the princess or what?

I don't think they know Gwynevere is fake, I think that they are protecting her.

they're like royal protectors but they're usually honorable and fight one at a time or w/e which implies that they both went hollow at some point

I cannot for the life of me kill titanite demons. I don't know what it is, I just have so much trouble with them.

the one in lost izalith can be just arrow'd to death if you really want to go about it that way
or try a plunging attack + rusted iron ring on the ones in Sen's
I personally say neither ending as good. Really, it's either letting the inevitable happen right then or have it happen later. The world's screwed either way.

First of the dead (note, not undead. Skeletons and hollows are different, you necrophobe) can also mean he's the most poweful, the top, number one. It doesn't have to mean he was the first to die.
The game doesn't say the dark sign dark sign is a new development. Also the Way of White believe in the Fate of the Undead and that the dark sign is cursed. People from lands other than Astora seem entirely indifferent to being undead, it doesn't bother Seigmeyer, Lautrec, or Griggs and Logan.

Of course it's not entirely clear if the age of dark is a bad thing either. In that way it's kind of like a metaphor for death, do you stick around and suffer, or venture into the unknown?

It never says it explicitly, but the rite of kindling item says "How peculiar that humans had found little use for humanity until they turned Undead."
It sort of implies that there was at least some time where humans had humanity, but there were no undead. If the pygmy's soul was the dark soul split into humanity, humanity was only around during the age of fire, but there were no undead, because it seems to be talking about humans as a whole. Of course, it may not be saying that at all, since it could be talking about individual humans. It would be so much clearer if they said "until they began to turn undead." I'm pretty sure they'd rather die than be clear about this though.

Just cleanly installed to start playing again
Start game, starts in a window, after a few seconds it crashes.
I verified and it said I was missing 1 file, and it downloaded GFWL. Still crashes.

I've been trying to invade someone in Sen's for almost an hour at SL30 but it always end up as Failed to Invade.
that's because you're ten levels too low. Invade sens at ~40.

Uh, invading doesn't have a level cap when invading upwards. Level 30 should be just fine. My Sen's invader is only level 35.
You might check your ports and make sure you've forwarded them.

I've been successful in the forest so far but I just want to know if there's a 'designated' area for people my level

I think that's being poetic, saying humans didn't realize the power the fragments of the dark soul gave them until they began to lose it.

You might try the Oolacile Township. That areas seems like a pretty good place for high level invasions.
