Oh God. I totally forgot about SHUMP's. rRootage isn't terribly hard, but Ikaruga is a BITCH.
Cave makes some wonderful SHUMP's. Mushihime-sama Futari is great, as is ESPgaluda II. DeathSmiles, which actually got a US release last week, is hard on the higher difficulties. Most of Caves console games have several difficulty levels and scale really well.

I'm playing the harder difficulty and shurikens and monks do almost 100 damage each hit. The control scheme is kind of funky too, but I got used to that.

The SNES version of Jurassic Park, never completed it, too tough (back in the days that is, I've never tried to complete it in years, maybe if I tried today it'd be a cake walk), either I lacked ammunition, or some freaking raptor came out of the bushes and I could never escape them, that or the damn T-Rex, can't avoid him either, or why not mentioning the interior levels (very basic FPS mini-game à-la Wolf-3D), those were very long, puzzling and difficult because all the damn corridors looked the same and and the raptors ambushed everywhere. I have both good and bad memories with that game, I liked it, but it was just too damn tough (also I never owned it, I just rented it a few times, like three times maybe, and then forgot about it). Additionally, Flashback, never finished it, I always ended up jumping right into a bottomless pit between platforms, or I would die to some completely subtle environmental hazard.

I thought that Prototype on the hardest difficulty setting was really challenging.
CoD 4 on veteran, but mainly because of all the grenade spamming.
I also find Lost Planet:Extreme Condition really damn hard no matter what difficulty I'm playing it on.

If you think that's bad, you should try out Red Tide. Even on easy it's highly difficult.
I know someone already said Super Ghouls n' Ghosts, but how about plain old Ghosts and Goblins on NES? Game's hard as hell, and just when you think you've beaten it, NOPE. Gotta do it again. My dad said that he stayed home from his drunken dad's wedding to beat Ghosts and Goblins and when he realized he had to do it again, he actually beat the shit out of himself. Like, smashing the cartridge into his face and shit like that. True story.
