I hate when my character speaks for himself. Like in crysis - in jungle, waiting for heli to approach, Nomad notices korean guys in these super suits - AND HE SAID "DON'T PICK ME UP, I'LL DEAL WITH THEM"
and dealing with them was so hard it was fucking annoying
That you have no feet/legs what-so-ever in First Person Shooters.
Random CTD`s for no real reason, i cant play fallout 3 anymore because it crashes every 30 minutes.

Well most of the time that's because of perspective issues. A lot of times it looks better not to have fee. But, if more companies could pull off body awareness like Mirror's Edge or Zeno Clash, that would be sweet.
And quicktime sequences that suddenly jumps out at you during a cutscene, Metal Gear Peace Walker style.
No subtitles in games, it can sometimes be quite hard to listen to a voice during a violent fire fight or something.
Bad ports of console games for the PC.
Useless "exploration" parts of games where you have to run around looking for some random thing that gives you a small xp boost or something. Not counting RPGs ofcourse.
