The most that the game will get is a rewrite in a new language. It's not a game that can really have a sequel since the protagonist is guaranteed to die by the end of it -- plus any additional content would be better off as an expansion like WotL was.

I think the only direction Isaac can go right now, is tacking on a shaweet dungeon creator that lets you make dungeons in any layout you please, and quite possibly even select a boss for whatever "Tier" the dungeon is created in.

Of course, there would be a steam workshop also.

Eurgh, I got Epic Fetus in a secret room, and Book of Revelations from a shop while trying the Purist. The health bar showed loads of soul hearts, so I tried donating blood for some more money, and it killed me. Apparently those soul hearts were fake

Blood machines don't collect soul hearts. (Unless you have 0 max hp)

If you still had normal hearts containers then the blood machine will take those instead of your soul hearts.

Yeah I thought they collected your normal hearts until you had half of one left. Then it moved onto soul hearts.

Did this happen in the Womb or any other floor that takes a full heart when you get damaged?
It's possible to get killed instantly from a Blood Donation Machine if you have exactly one heart left. It'll take that last heart and ignore any soul hearts you have. Happened to me once or twice.
