Say, would a Guppy Challenge Run be a thing to consider? If so, which of the two Guppy Spacebars would be better to start with; Paw or Head?
Also, another thing I thought of; Isaac Boss Rush mode. On each floor, Isaac would face all the bosses of that floor one after the other (Basement would have Monstro, Larry Jr, Duke of Flies, and Gemini/Steven), with each boss beaten yielding an item, and the first room of the floor after going down would have a few items (hearts, a bomb or two, and maybe a pill or trinket as well) to help with any losses made during the previous floor's Rush. After finishing the Womb Rush or Utero Rush, you'd be faced with three ways to go; into the Devil Room to fight Satan, into the Angel Room to fight [sp]Isaac[/sp], or through the Gnarled Doorway to reach the final Rush; the Dark Forest.
It seems that instead of the midpoint on the journey of life, Isaac found himself in the Dark Forest at a young age, where the true path was lost after his dad left. It is in this tangle of dead wood and grey brambles that Isaac would face the Harbingers, otherwise known as the Horsemen, one after the other; Famine, Pestilence, War, Death, Conquest and the Headless Horseman, before facing The Man-Willow, a massive tree adorned with nooses that requires all of your combined Isaac-ing skills to defeat, even with the Level 4 Meat Boy you got from killing the Four Horsemen. The face on the tree would also seem disturbingly familiar... (here's a tip; it ain't a Game of Thrones reference)
Obviously one problem with the Boss Rush mode would be that normally the bosses would drop their Items after being defeated, and defeating all the bosses in a single Rush Floor would leave you kinda powerful by the end of the floor, unless Spacebar items popped up as regularly as passives; that way one wouldn't be stacked with Collectibles. An alternative could involve a random chance of the Boss not dropping their item, though that'd probably leave the player feeling a bit "jipped". Then again, maybe the sheer task of fighting only Bosses one after the other would pose enough of a challenge without losing the potential drops to boost your power to survive the next floor.

Guppy's Head, definitely. And Hum, that's an awesome sounding mode. Maybe to solve the "Item Problem", he's given X coins per boss which he uses at the end of all of them for that floor in the "Rest Stop" to go shopping. The Shop is expanded; To conserve item space, the Flame and Shopkeeper are missing, with only items in their places, probably with 8 items total in the room. There'd be any number of Passive to Active Items, but they'd get more expensive the better they were, and the really, REALLY powerful ones would appear way later. Though, there'd still be XX% chance that an item would be a common item; be it Trinket, Pill, Tarot, or Bomb's/Hearts, though they'd be way less expensive, I.E. A Bomb is a Penny, A Heart is 2, Soul Heart is 3, Trink/Pill/Tar would be 6 and you'd be able to see what the Tarot Card is before buying it.

Monstro's like "I think I just shat myself"; reminds me of that one scene in Alice: Madness Returns where Alice ate the cake and the Royal Executioner dropped his scythe in awe and terror as she supersized.
Whats worse is after every successful run, you get a new trinket, lessening the chance of finding the Polaroid just a bit more each time.

turns out there's a limit on how many you can have so when you pick up a new one it just doesn't appear
The Polaroid should [i]really[/i] be set to have a 100% chance of existing somewhere in the Cathedral for every run. That way it's still rare to encounter if you're just buggering around, but if you want to reach the chest you can always put in the extra effort and resources required to intricately explore one of the most difficult floors in the game.
Hell, maybe even set it so Daddy Longlegs is always in a Cathedral room and [i]he[/i] always drops the Polaroid. Family reunion!

Having no true heart containers is indeed tricky; with the other characters 6HP is ok-ish since you can eat the red hearts, but with Blue Baby your regenerative capabilities are dependent on more values; Spirit Hearts are less likely to drop than Red Hearts, though having bombs can help one get 2HP-4HP from the Tinted Rocks (1-2 Spirit Hearts) if you're lucky, and there are of course items that can give Spirit Hearts like Book of Revelations, which would synch pretty well with the Nun's Habit.
Blue Baby's more of a challenge run than a true class in my opinion; Isaac's the Jack of all trades, Maggie's the Tank with extra starting health, Cain's the Rogue with luck and speed, Judas's the Mage who's like a glass cannon, Eve's kind of a Bloodmage (take damage to summon a minion, turn into a demon when on her last legs, Razor Blade sacrifices health for extra damage, etc), Samson's the Berserker who gets stronger as he fights, but Blue Baby doesn't seem to have a true role like the others do.
Speaking of classes, what other kinds of characters do you think would fit alongside the current roster? I'm thinking of a hit-and-run "assassin" kind of character, starting with low health and a slow rate of fire, but also starts with tears that both do damage over time and slowing, as well as a Spacebar item that allows him to essentially "vanish" from enemy sight whilst reducing their speed, meaning that players would need to employ hit-and-run tactics to take out enemies, damaging them and then sneaking to a better vantage point to strike again. The aforementioned "Moon Cloak" would likely recharge in a similar fashion to either the Candle or the Bean.
I'm thinking the "assassin" would be called Mark, after Marcus Junius Brutus, one of the assassins who killed Caesar; the character's cosmetic indicator (like Eve's goth getup, Cain's eyepatch and legband, Judas's fez, etc) would probably be that sort of leafy headwear associated with Caesar and Roman politicians. Wasn't the assassination of Caesar in one of the later versions of the Bible or am I not thinking straight? I'm probably thinking of Dante's Divine Comedy, since Satan was chewing on Brutus and Cassius (and Judas) around the end of the Inferno sequence. Regardless, an item that would probably synch well with Mark would be Mom's Knife; the short-range stabbing power not only suits the playstyle, but also the aesthetic of the character, and perhaps when Mark has Mom's Knife, the "Mom's Frock" that characters usually end up wearing would be replaced by a toga of some sort.
I would love a character that starts with flying, pretty much.
But be really squishy and all.
