I was pretty pissed when, after beating the chest with judas and coming back to it with ???, I only got the new ending instead of being taken to the actual chest floor.
It seemed pretty much granted to me that, every floor you've been to is by unlocked by default. Guess that was asking too much. Add insult to injury I already had beaten [sp]Isaac[/sp] as ???.

If someone can make it all the way to Isaac and win with an utterly terrible build, they could at least get the opportunity to [i]attempt[/i] to beat the Chest. That additional floor is part of the journey, to restrict a large number of players from even attempting that part of the journey to chance and performance is ridiculous -- beating Isaac alone should be good enough proof that someone can perform and should be granted an attempt at beating the Chest. The Polaroid-exclusiveness just doesn't make sense.

I completely fail to see how finding a certain trinket completely by luck has anything to do with performance.
I am fairly confident I have achieved the shortest run possible
Went with Judas, spawned right next to the leftmost door in the starter room. Instantly held left before screen faded in fully.
And as soon as I get through the door, there's a lit mega troll bomb 3 blocks away from me, having spawned inside some rocks. I'm fairly confident it would have been completely impossible to run away from it.
In all, that run lasted less than 3 full seconds. Maybe 2. Frame-by-frame, I don't think it's possible to die faster in any way.
I laughed pretty hard.

Beating Mom's Heart quickly has no bearing on whether you can access Sheol or not. It functions as a Devil Room, and the Devil Room odds of appearing are based on how much damage you've taken that floor, whether you've got a Book of Belial or Book of Revelations, if you have killed a Beggar at any point in the floor, if you have a Pentagram, and other things.

I believe taking no damage during the boss does increase the chance of the devil room quite a bit, though.
I'm starting to feel like I dislike the way the game is developing. Adding the chest on top of the new area makes the game a little longer than I'd like it to be. It's hard to believe that at one point it was just basement, caves, and depths.
And the game is definitely getting harder with this new content added, but a lot of it is made harder in bullshitty ways. Like bloat being able to instantaneously fire brimstone with no warning, so pretty much all you can do is shoot him from behind (which isn't always an option). Or Conquest firing four really fast homing shots that are really hard to dodge, even with speed upgrades. I also passionately hate how a few random enemies can fire homing shots, with no visual indication to know which one is going to do it. Even worse when it's one of those little angels, not only firing homing shots, but three of them at a time.
But the homing shots wouldn't be so bad if they didn't occasionally take way longer than normal to hit the ground. I'll often think I've dodged one, but then it just keeps coming at me.
Also, regarding the fight with [sp]Isaac[/sp]: [sp]It gets to be way too much of a spam-fest of homing shots, when both Isaac and his minions are firing them at you in large bursts. Even bullet-hell games usually have things fire at you in a set pattern, not following you wherever you move. And I'm sure this has been said before, but you shouldn't have to kill the angels after Isaac is defeated.[/sp]
Maybe I'm just not used to things though. Maybe after playing it a while longer, I'll feel less like the game it out to get me the whole time.
I think giving those screaming baby things the power to shoot Homing shots was really unnecessary.

I'm fairly certain that it was a bug. Consider that only 1 of them ever gets homing tears, and it's only that one that spawns in the bottom-most and right-most location.
I fucking hate Rooms where the blue spike things charge you as soon as you enter.

Enemies that attack with little to no warning annoy me.
The ones that come to mind are Gurdy JR, and Bloat.
Gurdy JR will fling herself at you with no warning whatsoever, and Bloat fires instantly.
A tip that no one seems to know is that you can hit the Bloat from the sides without being hit by his Brimstone if you stand near the bottom of his body. The Brimstone shots will end up going right above you.
Mutant Spider is such a god damn life saver.

He's just fucking annoying. Especially the head. When you enter a room and there's 3 of them and 2 start charging in opposite direction while the last one shoots fucking triple shots at you, you just feel like going "fuck this shit".
Oh and it's always nice to encounter him as a boss on Basement 1. As Judas, too.
Heck, even the item he drops is bloody useless if you have a spacebar item that you don't want to lose.

So, uh. Remember that invincibility glitch Edmund fixed where you have no hearts but you don't die and you can no longer take damage? I just did that by accident in the first room of The Chest and I've had infinite life ever since!
If you use Blood Rights and have one heart left, it'll bring you down to none and then you'll be invincible! This next patch is going to be like the third separate invincibility bug he's fixed, hooray!

I find it silly how Habit, 7 volt and Battery improve your charge by set amounts. Works disgustingly well with slow-charging items, doesn't help much with fast-charging ones.
They should charge one "tick" for your item instead. Like, if your item recharges in 2 rooms, using it with the 7 Volt would leave it at half charge, and if it recharges in 6, it would leave it in 1/6th.
Same for Habit, and possibly Battery could give you only one time-set "tick" instead of a gradual charge (which potentially worked as 3 "ticks" for slow items)
End result, you now can't use the Nail once per room if you're a munchkin, and you can now use things like Mr. Boom every room. Maybe even use Tammy's head twice in the same room, why not?
Pretty sure this has been probably thought of already, just tossing my two cents.
My game still slows way down for about three seconds every time I enter a room, regardless of what's in it, or whether I've been in it before. It's every single room.
This has been happening to me since the DLC, but I think I'm the only one.
