I was going great, had all sorts of great shit. I beat the boss on the floor, and went back to gather shit. Had half a heart left, so I went back to the shop to buy heart. I accidentally hit the fire. And died.

IMO this game is like playing a slot machine. Basically you get good items and kick ass or get shitty items and lose. It's skill, yeah, but largely luck of the draw.
I mean, I like it and I can appreciate its draw, but in order to really enjoy it you need to accept that sometimes the game just wants to give you The Virus rather than anything useful.
Wouldn't Virus work well with Book of Shadows or Unicorn, though?

Aye, most spoilings here don't matter anyway, don't they? It isn't like the game is steeped in masses of lore that people want to find out for themselves. To me at least, apart from the few cutscenes in the game, the narrative of Isaac is usually the same after the first few times you play through; Isaac goes down into the depths beneath his house, fights his semi-aborted brothers and sisters/manifestations of his fears and dislikes, grows in power as he gathers items, confronts his mother twice, then is faced with a choice; either ascend to the Cathedral and fight his true self, or descend into Hell and confront the devil.
But the items and characters themselves seem like narrative too; many of the original items relate to how he was abused, like the "meals", the belt, the coat hangar, etc.
Also, concerning how the Chest is essentially the Cathedral Part 2, there seems to be almost no reason to descend into Sheol nowadays; it used to be "I must defeat the Satan!" and people all descended into Sheol to make their final "deal" with the Devil. Nowadays it seems like everyone's forgotten about Sheol after defeating the Devil enough times. Personally, to equalize the massive disparity between Sheol-divers and Cathedral-crawlers, there should be a level after Sheol that is a sort of "demonic analogue" to the Chest; the Forest, where the true path is lost.
Maybe having a certain trinket, descending into Sheol and defeating Satan carrying that trinket (or maybe defeating Satan as Guppy), would result in the usual Chest being replaced by a trapdoor adorned with thorns. The boss of said forest could be a large tree that consumed Isaac's father, but once defeated it turns out Isaac's dad hung himself after things took a turn for the worse, then Isaac finds himself surrounded by dark shadows that close in on him.
Turns out that in Isaac's madness, he managed to escape the house and ended up in the forest, and was later found by social services. Isaac's mom loses custody of her son and is arrested for child abuse, whilst Isaac ends up in psychiatric care after the horrors he faced. Isaac's doctor, Professor McMillen, is disturbed by the drawings Isaac drew as part of his therapy; the boy seemed to recall his adventures as a harrowing journey into the depths beneath his mother's house, as if he were descending into Hell itself. It is uncertain whether or not Isaac ever recovers from his ordeals.
Meanwhile, a further investigation of the house led to the discovery of a trapdoor, and as one of the cops shines his flashlight in, he gasps and the scene ends; we are left uncertain as to whether Isaac's ordeals truly extended beyond his mother's abuse...
This way, the choice between Cathedral and Sheol wouldn't be as lopsided as it is; you can truly choose how it ends, either with Isaac accepting his fate, or ending up in psychiatric care; either way the endings aren't exactly happy, which after all is one of the game's main themes.
