I hate that, too. Though, it did force me to miss 2 item rooms during a terrible laboured run as Magdalene, which forced me to unlock Samson by complete chance.

i know right you can be unlucky in a game based around luck that so terrible
The part based around luck is more based on what items you can get and how powerful you can get, it seems kinda stupid to downright limit your ability to make a decision by the simple fact that you can't have the requirements for it.
I just don't think that you can't even get to the point where you rely on luck because of some stupid restriction of not having the item and it not relying on skill in any way, shape, or form, atleast with items you have the chance to turn a shitty item good.
You know getting screwed is part of the game

Makes the game harder.
Less health drops, more aggressive and colored enemies, bosses have more health

Various things. For example decreases the amount of health pickups bosses drop after they die. Also makes champion enemies more probable to appear and makes alternate (colored) bosses more probable.
With more Champions, there's more risk-reward from what i've seen; if the Champion is killed, they usually drop something extra, so if you aren't rubbish it's essentially more drops.
But it was already hard enough :c

Killed It Lives and got a Cathedral entrance, a Sheol entrance, and a trap door. Anyone know what that trap door leads to?
Trap door is Sheol. You're probably talking about the devil room when you said "Sheol Entrance".

Devil/angel rooms on womb/utero 2 send you to Sheol. It's normal. It helps you if you're at <9 endings, but otherwise it's useless.

I always regret picking up toothpicks because the amount of cringe in the playthrough is not worth the bonus.
If you cringe at toothpicks, don't google anything in this game that you think might be a medical reference.
Terratoma and fistula, for example. Or even so far as the utterly gut-punching harlequin baby.

Ugh. Had mom's knife, doing an awesome run as sam. Died in the chest though. I swear, every run with sam I either get lots of damage, or lots of hp. Never get both, or a middle ground. When I have high damage, I die too quick, and if I have lots of hp I do great until I run into some boss that doesn't spawn any minions so I can get my rage going. Not cool, sam. Not cool.
If we are talking about this bizarre stuff: I keep wondering about the story - Isaac runs away from his mom into the basement (Basement/Caves/Depths), but she finds him (Mom fight), he manages to stun her, then enters her vagina (Womb) and then kills her from inside (Mom's Heart fight) and then [sp]suffocates and goes to hell/heaven[/sp]. I don't even.

I always just imagine it as a dying dream Issac has while his mom kills him. :v:
