It's bugged.
Give it some time before things get fixed. Halloween patch was buggy like this too.

So I beat the final boss and [sp]got the d20 and what appears to be a piece of a photo[/sp] How do I get the others?
he starts off with low everything, but bloodlust brings your attack to max by the end of a room
on bloodlust: it only counts per room, not for the entire game, so it's kinda shitty on bosses
finding mom's knife then tech 2.0 after shoots a projectile that does nothing
you can still use the knife, but you shoot a small laser as well

Bloodlust is great on bosses that have summons. Which, as it turns out, is a great deal of them.

Man I had the most OP'd combo just now. Max damage, Technology 2, Sacred Heart, Goat Hoof, Blood Lust, Tough Love, Book of Belial, They Come From Inside (generates friendly flies that suicide into enemies for insane damage), Holy Grail, Spirit of the Night, Little Gish, Little Steve, Ouija Board, Cube of Meat and Halo of Flies. I beat the final boss in less than 20 seconds.
The other bosses spawned in the final level were taken down in 5 -10 seconds depending on the boss. Widow went down in 4.
I had the most OP as fuck combo
Fetus in Jar + Holy Grail + a plethora of health upgrades as well as Ouija Board, and ICEPAC

Spectral bombs that also did poison damage, MOTHERFUCKERS

You can't pick it up because you already have The Tick. The Tick keeps you from picking up any trinkets I've noticed.
