I went in with only 2 hearts and ended with 1. Had to do some crazy dodging to stay alive when I aimed the Epic Fetus.
Either way the spider challenge is dealt with..Now only got the Purist Challenge and all the Challenges will be complete. Then I move on to unlocking more items..
How do you unlock puzzle pieces? I've killed the final boss already four times and still got no pieces :c

Assuming by final boss you mean [sp]Isaac[/sp] then you should already be unlocking parts of the Polaroid, it can be assumed that you indeed are unlocking each piece but a bug is refusing your progress to appear on the title screen, as a couple others here have said the exact same thing and they found the Polaroid eventually without the title screen showing the proper image.
I just beat [sp]Isaac[/sp] for the first time, saw the new ending, yet it's not unlocked in my Endings, there is no Ending 12.

Yeah I unlocked the Polaroid today but my title screen is still golden god
My challenges have screwed up, I can't unlock challenge 10 despite having [sp]beaten the Cathedral several times (as Isaac, Samson and Magdalene), which is what the wiki claims is required to unlock it (although they do also mention that this is a known bug, but offer no fixes)[/sp], anyone know how to fix it?
Not sure if it is related, but I have also found the Polaroid on several occasions, despite having not [sp]compled the Cathedral as all characters yet[/sp].

You need only to complete the Cathedral a set number of times, it does not matter if you complete it 5 times as Isaac or a mixture as a range of characters.
Which raises the question, did anyone count the number of runs the Polaroid took to complete?

Loads of people have that bug including me. It relocked in the 1.15 update and I don't know about a fix.
IPECAC + Mirror = Goddamit.
Run ruined.
Beat [sp]???[/sp] while holding the Polaroid just to be sure that I didn't miss that. Still have 86/89 secrets; I guess I will get stuck at 88/89 until the notifications get fixed because I have no clue what that last one is unlocked by and apparently no one else knows either.

Yes. Absolutely.
Just had my first Judas run and it went fucking fantastic. Got Epic Fetus on Basement 1, by the end I had 3 normal hearts, soul hearts off the screen, flight from 3 separate items and a 1up, unlocked ??? on my first attempt.

If you are lucky enough to find Wafer + Book of Revelations + Nun's Habit, you'll practically be invincible.
I have just found Technology 2, Pyro, Skeleton Key and a bunch of Damage boost items in the first 3 Dungeons.
I like where this is going.

I have far better luck playing as Isaac and finding Bloody Lust in the first Devil Room. At least that way I don't have to deal with an abysmal fire rate.
Quick question on a combination of items: If a have the Inner Eye (Three tears) and I'm about to pick up Cupid's Arrow, do I lost my three tears? And does it still count as tears, for the bonus?

Holy SHIT I am SUPER lucky, I just beat [sp]Isaac[/sp] for the first time ever.. AS FUCKING ??? (Also my first time ever getting the [sp]Cathedral[/sp] after it lives. I got the epic fetus AGAIN which made it super easy, got a prayer card too which helped, would've preferred the Book of Revelations but hey, it worked.
Samson is such a bad character because I find bloody lust pretty much EVERY game with the other characters.

Should I pick-up it? Playing with samson and having mooligan + quad shot seems quite useful.
mooligan + tech 2 is deadly tho.

I wouldn't imagine so, as Cupid's Arrow gives you piercing shots I think. Shouldn't effect that.
