I guess I should've known better that most people just rush the game with no galactic readiness points and screw themselves at the end. Doesn't really explain why garrus and vega didn't die just as much during that moment, considering they were the most used squadmates after her.

I had tali die on my final mission Whoops lol (I totally went "fuck no" to her wanting to be exiled) On that note is it possible to sex up the gay shuttle pilot in ME3? I'm thinking about actually doing only paragon choices for him if I can actually get away with it lol

If we're talking ME1 biotics, you're probably thinking of Throw because that power is insane.

First run of ME2 after ME1 import. Did all the missions and assignments I ran into, but lost two people at the suicide mission still. :(

Example, send legion or tali into the ducts, not jacob.

I always send mordin to guide the crew back

I guess the fault was just with my choices as to who should follow me. Also the crew didn't make it, all my regret.
Samara and Jack I think are the ones who are supposed to do the barrier. Always send Mordin back with the crew. Always go get the crew right after they are kidnapped if you want them to live.

Each character has a combat rating, the overall combat rating of the group holding the door determines how many live. If it isn't high enough, characters are killed off based on their rating, lowest being most likely to die. Mordin has the lowest rating I believe, which is why he's the one who tends to die. It's only really a problem if you pick the guys with high ratings to fight the human reaper with AND don't have DLC characters. For example, Grunt has the highest rating I believe, so taking him and someone else like Garrus really damages the rating of the group and puts weak characters at risk. However, with DLC you have Zaeed and Kasumi added into the mix so that rating is going to be higher. Sending him back with the crew as an escort is the best way to secure his livelihood. The next best way is to avoid taking the obvious "tough guys" with you to fight the human reaper. Don't take guys like Grunt, Zaeed, or Garrus with you, even if everyone is alive you risk losing someone due to wrecking the combat rating of the group.

I recall my horror as I saw Garrus being carried away screaming on a cloud of Seeker Sawarms. But now you know for next time! [sp]Or you could just restart the mission[/sp]
I made Legion go through the ducts, let Jack do the Barrier, sent Mording back with the crew and took Tali and someone I can't remember with me to the reaper.
