Spore, kinda.
i can't believe only 1 or 2 people have so far mentioned Just Cause 2. Great game, as are X3:TC, all the R* games and RF:G.

If you can stand not having voice in all conversations then I'd say the best sandbox game (by OP definition) would be Morrowind
GTA4 It's HUGE!!!
Red dead redemption is as large if even bigger with more variation in enviroment and alot more to do.
Just Cause 2.
X - Beyond the Frontier with it's expansion pack, XTension. If you can't find it/it's too hard, then X3 Terran Conflict. [editline]06:30PM[/editline] Which X3? Reunion or Terran Conflict? Pisses me off when people won't specify which one :colbert:
Terran Conflict Better interface, more ships+weapons, more sectors, competetive pirate arsenals, better mission system, easier to get into - fixes or works around problems in Reunion and fixes gaping holes in the interface
Saboteur, the nazi shooting GTA. It was fun to explore Paris and to jump from the Eiffel tower.

The other Elder Scrolls games, Saints Row 2, Crackdown 2, and Gothic 3 are all good.
Destroy All Humans 1 + 2. Not the third though, or the one on Wii though.
I would say Spore, Fallout 3, Jak 3 if you have a PS2.
red dead redemption ?
red dead redemption ?
I don't really find that an Open World type game. More mission to get this mission to do this, mini game for money.
Prototype is fun, has a few shitty bits though.

You got the entire gamespace to play on whenever you choose at any one time Also, Terran Conflict if you can run it, if you can't then Reunion
you can never go wrong with the gtas and saints row do that matter

Although you already mentioned it, Fallout 3 is probably my favorite sandbox game. Buy the PC version if you haven't already and install some mods from FalloutNexus. Endless hours of fun, and with the right combination of mods, very long lasting as well.
Glitchy game, shit story, and annoying wrecking physics.
You say that in a way that implies GTA4 doesn't suffer from the same issues. You cannot deny this but I know someone is going to try and convince me that GTA4 had a good story. Try me.
I looked up X3 Terran Conflict. Looks really good.
anything from Rockstar Games
Even Lemmings?
I don't really find that an Open World type game. More mission to get this mission to do this, mini game for money.
You have 27 square kilometers to explore. How is that not open world?
It's open world maybe, but it's not a sandbox.
Eve Online.
Anything the elder scrolls.
It's open world maybe, but it's not a sandbox.
To the appropriate extent it is. It allows for the player to do whatever he/she wants, the player may choose what missions to do, and the player changes the game based on certain decisions.

It just wasn't as groundbreaking as everyone thought it was.
