In no order or relation to the upgrades, Jack gets blown up, Legion/Tali gets evaporated, and Thane gets hit with a I beam and crushed.
I had all upgrades as well.

Have you tried taking samara as the barrier? She should be fine.
I used Samara for the barrier. So that may help.
Use Samara for the barrier. And send Doctor to get the people back to Normandy.
Just like what the others said, choose Samara for the barrier, she will succeed. [sp]UNLESS YOU FUCKED THAT UP AS WELL.[/sp]

I had everyone survived, I had Everyone's loyalty except I lost Jacks in the argument with Miranda, Samara made the field, worked fine for me.
There's actually a table of who to use to survive the suicide mission.

Tali led a team adn they all defected and died, Garrus' guys all died, Jack was with a gang and they split up and got killed, Samara doesn't work well with others...
Game has a more climatic ending when Shepard dies.

I always pick Garrus to be the fireteam leader. he's a natural leader

Zaeed is a better example, if you listen to his stories every time he's lead a squad the whole squad died.

Side note Garrus didn't get anyone killed for me. It comes down to how you talked to them/if they are loyal and where you put them. No "guide" or "table" is going to tell you how to play and win. You just have to play and figure it out for yourself.
