If you didnt upgrade then you would lose people in the initial jump, not later. Different upgrades save different people. In no order or relation to the upgrades, Jack gets blown up, Legion/Tali gets evaporated, and Thane gets hit with a I beam and crushed.
I had all upgrades as well.

It's largely chance and how you talked to them.

Have you tried taking samara as the barrier? She should be fine.
I used Samara for the barrier. So that may help.
Use Samara for the barrier. And send Doctor to get the people back to Normandy.
Just like what the others said, choose Samara for the barrier, she will succeed.

Jack is capable but not in this case.
I had everyone survived, I had Everyone's loyalty except I lost Jacks in the argument with Miranda, Samara made the field, worked fine for me.
There's actually a table of who to use to survive the suicide mission.

Or common sense and a good memory, the game tells you several times who not to choose to lead the second team for example. Tali led a team adn they all defected and died, Garrus' guys all died, Jack was with a gang and they split up and got killed, Samara doesn't work well with others...
Game has a more climatic ending when Shepard dies.

His team died because they were betrayed, not because of bad leadership.
I always pick Garrus to be the fireteam leader. he's a natural leader

That's because Zaeed is a hard ass and sacrifices whoever is needed to get the job done....oh wait I just described my Shepard.

I had Garrus lead them and Jack died :happy:, but I only just downloaded Zaeed, I don't trust him with my fireteam.

Garrus (along with Miranda and Jacob) are the "natural leaders" of the pack and should always be put as the leader. Side note Garrus didn't get anyone killed for me. It comes down to how you talked to them/if they are loyal and where you put them. No "guide" or "table" is going to tell you how to play and win. You just have to play and figure it out for yourself.
