I remember back in the days the PS1 and nintendo 64 games, all of them were like 15 hours + and hard by default.

Igni And Rudra on stage 5 in Devil May Cry 3(Not special edition) Was always hard for me without using items or retrys. I can never imagine doing it in Dante Must Die mode. Beowulf is a really close second.
No Fighting in the War Room in CoD4 and Heart of the Reich in WaW I still haven't beaten either of them.

Especially because it was my first playthrough and was on advanced... The last part of the final WaW level was also annoying, just because there were guys everywhere and the AI didn't help.

You could have full health and awesome armor but if you take a wrong turn or slip off an edge/get hit into a long animation, you're totally fucked. I've been killed by one guy because of shit like that. Especially in 5-1. Motherfucking far cry 2. It wasn't that hard really, but it's just the fact that you save before attacking a large enemy camp, make it out alive w/o syrettes and little ammo. Then as you're driving back those africans are suddenly in cars and the last of your ammo goes to get them off your tail. THEEN you have to fight the respawning checkpoints enemies (it's plausible that you passed at least 1 checkpoint whilst heading for your goal), so I charge in with two molotovs and a machete, fuck some shit up. Get shot to shit, and not having any friends to rescue you. Well at least my autosave will let me resta-- oh right. Fc2 doesn't autosave. And then you exit the game by smashing your keyboard. It's sad, because the game is so beautiful and well-made otherwise. [editline]05:32AM[/editline] Excuse punctuation and grammar.

The Dragon Age Origins DLC Golems Of Amgarrak. The Harvester on Nightmare Mode. My Warrior had a horrible setup and got his ass handed to him.
I think the level in Tomb raider where you had to fight off the Tyrannosaurus Rex was pretty hard... Although the traps level was insane X_X
Defeating Matriarch Benezia in Mass Effect at like level 15. Never raged so hard in my life ever. I will never make that same mistake again.
