No Fighting in the War Room on Veteran in COD4. Extremely narrow hallways, tons of enemies all behind cover, pretty much instant death as soon as you peeked down the hall, and the enemies would respawn indefinitely, so you had to be incredibly fast too.
Also, the entire COD2 campaign on Veteran was ridiculous.
Like every single second in enemy territory when playing MGS2 on European Extreme mode... especially when fighting against the bosses (vamp, fucking bastard).
The easiest were the 25 MG-Rays, when you know how to dodge their stuff. It's easy then, even on EE mode.
The Skymall panic event in Diescraper. It was so annoying when after ten minutes of being spammed by zombies, I realize I have to turn off an alarm.
Majora's Mask itself if you don't have a guide or walkthrough with you. That's why I'm glad I found NintendoCapriSun. His Let's Plays are great walktrhoughs.
All of the Follow but do not get noticed, escort, and catch up with him/shake them off your tail missions in GTA games.
Not all of them hard but fucking annoying.

I despise the grenade spam that goes on in the veteran difficulties of the CoD games. I really hate that CoD developers think avoiding grenade spam is indicative of a highly skilled player, as if increased damage wasn't enough.
Episode 3 : Chapter 3 of Lost Planet 2. That fucking worm... Wasn't so much as hard, as it was purely annoying. Killing that thing requires teamwork and normally no-one helps.
That one sidequest in Spyro 2, I believe it is, where there is this big tall annoying guy who claims to be a spy, and he'll only give you access to his house if you follow him without him noticing you.
Near the end of the mission he starts getting a little paranoid and turns around after each step or so. If he leaves your sight for even a second he tells you that you failed, and if he is even close to looking in your direction, he tells you that you failed.
It was just so unnervingly difficult.

Is it easy? No, but it's far from the hardest.
Ninja Gaiden Black on Master Ninja, every single mission is harder than any Halo on Legendary, they'll make Demon's Souls on NewGameN+ (which is hard) seem like a walk in the park (though DS is more of a game of patience, a different kind of hard). Especially when you get to the final boss without health elixirs.

I beat her pretty easily with a PSG-1 on Hard. I think the torture scenes in the MGS series are way harder than any boss. FUCKING BUTTON SPAM!

In Mass Effect when you had to find Liara, where you encounter the Geth armature on foot. The armature itself wasn't too hard to try and take down, but it was because both my squad mates who were low on health and barely did anything but die that made it difficult.
