Supply Lines isnt it? Oh god... There is only one word that can describe that mission. >_>'
*Sigh* I was young when i done this, So i cheated with Action Replay max for this game... But even action replay cant do much for this since its a bar and not a time limit which you can freeze.
The most recent one for me would be MGS: Peace Walker's (Using Spoiler Tags for this one, If you like surprises while playing) [sp]Tortue Sequence... Oh boy, This fucker comes back with a vengance after its absense from 3 (Which you dont need any button Mashing to save yourself) Portable Ops (In which there isnt any Torture Button Mashing at all) and MGS4 (Which i dont count, some people do count it as a tortue sequence near the end at the microwave hall way), Call me a noob, but i cant get a hang of Mashing the Triangle Button on the first generation PSP to save myself, its just not comfy on your fingers. X is a fine button but Triangle is just a curve ball...
And i've not been trained to button mash to survive as much unlike some people i know, I had to give my friend the controler for MGS1's tortue sequence because i would take too long and we didnt have all day. xD
Luckly, And i thank kojima for this, You can do this mission on Co-Op's and the second player can sneak out of their cell and shut down the power for the whole building, cutting the Tortue Sequence for the first player. But if you had no one that had Peace Walker, this one mission would be that damn level for a long time to come...[/sp]
Oh yeah, I would say lots of Parts in a MMO called Mabinogi, but that all depends on who your with, You knowing what is coming behind that door and around that corner, and how well you can fight in the game... The game is Evil, i tell you. (And the strange thing is, You cant say "This game is evil" in the chat box. It censors it. xD )

This one right here, that is, if boss levels count.
Bionic Commando Rearmed final level.

It's pretty easy in co-op, but pulling it off in single is hard as shit.
Hogger.
Every fucking level on Alien Hominid, try not dying.

I deemed that level as impossible since whenever i fucking take out the vans, by the time i reach the landing pad the gas always runs out RIGHT AT THE FUCKING END.
Can't believe nobody has said the Temple level on Goldeneye, that shit made me go insane
Animal Antics

The fucking part where you have to get through a shitload of brambles with squawks WITH WIND CHANGING WHEN IT FUCKING FEELS LIKE IT
The Bramble Blast track brings a tear to my eye every time I hear it for some reason.
Geist. The whole fucking game.
In one mission you have to destrpy like 10 AA-Guns in a very short time while there are enemys everywhere
:colbert:
[editline]05:59PM[/editline]
Also in GTA: Vice City where you have to fly that toy helicopter.

Reversing controls, pinpoint timing and twitch reflexes needed, all that kind of mind-bending shit that just drives you insane.
Halo? Seriously? How was ANYTHING in that game even remotely difficult? Fucking kiddy bullshit is what halo was... How anyone could have trouble beating any part of that game is a complete mystery to me.
I remember playing that with a friend a few years ago. Didnt know it had such an epic ending though, we stopped at some point coz it was too hard.

Lol Nexon and NX Store.
Timesplitters 2: Neo Tokyo on hard. 'Nuff said.
Nar Shadda Streets level in Jedi Outcast, I hate those fucking god damn snipers, and the Bathhouse level in Chaos Theory.
Does Kaizo mario count?
Waaay too many people posting MGS in this thread. Come on people those aren't the only hard games in the world.
Medal Of Honor Allied Assault's Omaha beach landing on Veteran is actually impossible, it seems like an oversight by the developers because you are meant to take a certain amount of damage as soon as you get out of the lander, on veteran you lose 80% of your health in a scripted sequence that cant be bypassed. Needless to say the snipers and machine gunners on veteran never miss a shot and you are gunned down again and again, even funnier is that they ignore the 30 other npcs on the beach they just go for you.
If anybody has managed to get to that mission on veteran, congrats that's as far as you can go.
Final boss of Bayonetta on Hard.
Dear god.

I think that's on normal though.
Notice "Part 1"
All of Crash Bandicoot 1. The old bridge levels were the hardest. I even broke one of my controllers from rage when I played it the first time.
Crash Bandicoot Warped, where you have the levels where you gotta ride your motorcycle and beat a bunch of fags in cars. If they ram you from behind, you slow down and they speed up. You ram them from behind, they speed up and you still slow down. The thing is they purposely ram you. I raged so many times, but I got the gold time trial trophies finally.
The swamp levels in stage 5 of Demon's Souls.
The level in Goldeneye 64, where you start out in the elevator with the dumb chick. Here's what happened when I played
I walk out, kill the three people right outside the elevator.
The girl runs over and opens up the door. I run through the door and after killing probably, oh, more than 50 people, get to the big room with the gigantic screen. Boris is standing there.
"Oh, please don't kill me. I'm just a lowly programmer."
He pulls out his gun, I shoot him. I go to the girl, and she yells at me and says:
"You didn't need to do that to that poor little programmer. I'm leaving."
Then she goes to the elevator and leaves, while I'm standing there saying What the Fu#k just happened?
So I restart the level, and do what I did up to boris, I take the gun this time,(follow him and get the easter egg) and she starts to hack into the mainframe. THIS IS WHEN THE SHIT HITS THE FAN.
Waves and waves of enemies come and start to come and just bombard me with bullets. This is why I can't beat the level. I always lose when I get here.

Also, all of Mission:Impossible The game on the N64.
Specifically the repeling mission with all the lasers. That was too hard.
The Tower of Lost Souls on Soul Calibur 4. To this day I have not beaten Level 36.
"All you had to do was follow the damn train, CJ!"
That god damn level. Fucking Smoke, you're completely useless you fat fuck. But you sound awesome and you ate all the cold food.
You can ramp off one of the hills on the side of the train tracks onto the train and proceed to run over/'forward aiming drive-by shooting thing where you shoot with one hand over the hand-bars of the bike' while on the train. Not easy to pull off, but it's a guaranteed success if you make it.
EDIT: Ugh my post is horrible to read, sorry, I don't know how to describe the shooting while on your bike thing
The whole game.
