I was thinking it'd be pretty neat if in ME3 the Reapers started projecting images into Shepard via his implants of either destruction going on elsewhere in the galaxy, or destruction from past cycles. You know, to try to break him. To make him feel like fighting is hopeless.

It was a save import bug, it's common for developers to make shit up (Bungie does this a lot).

I don't see how they could resolve it because both flags are checked in your ME2 save file if you dealt with Conrad in ME1.
There are ways to fix this, however:

I made sure to check it so only the paragon choice worked. And it still happened.

Check the video - the Paragon resolution dialogue is present and playable in ME2.

Try with the linked tool and these instructions:
0. (X360 only) Move your save file to USB stick and extract it using Modio (more details in "Modding is possible" thread)
0. Backup your save file
1. Go here: [url]http://b4rt.rootnode.net/masseffect/index.php/site/index[/url]
2. Register new account (no email activation required) and login
3. Open "My saves" tab
4. Upload your save file (.xbsav for Xbox 360, .pcsav for PC)
5. Click "Plot table" button
6. Untick "[Conrad] (3) Intimidated Conrad (Renegade)"
