They probably do care in most cases, but nobody knows what's actually being uploaded to the workshop until they sift through every page of potential additions to the game. You can't blame them for just wanting to have a clean game without the need to sort through a mess of an upload system opening, viewing and rating everything they don't want to see. It's the people who don't want a mess of cosmetics in their game that are the kind of people who won't visit the workshop frequently enough for it to matter, if they ever do. You really can't blame them for not knowing about things they will probably dislike if and when they get added. I know I've never seen or heard of most of these recently-added sets, and I really don't like the majority of what's been added recently, but I'm too busy either out of the house, doing work or actually playing the game to go to the effort of looking up all of these sets.

Generally only items on the first three pages of the top for the week are added anyway, so you can just casually check those every few days and have a good idea of the popular items and the ones with the best chance to get in. And rate down any you have problems with. You can't expect Valve to know what the community wants, when the venue they give to show what they want to support is ignored.
Another idea would be limited number of votes on the workshop for everyone (just like it is with reports and comments now). So now each vote would matter since we have limited number. Players won't throw them here and there but would choose wisely. Then only truly the best items would get in. For a game that works on 50% winrate in matchups, i think it's fair enough to mess up sometimes.
