It would be easily exploited and possibly used for griefing. For instance, a triggerhappy kid makes an SMP server, and permabans anyone who joins, thus making them unable to play SMP forever.

Not if it's controlled, eg has to be approved by multiple servers, but will affect that server but not global until approved.
Whenever I start a sever no one can connect, which sucks.

Open firewall, open ports on router and you're done. You've probably done something wrong.

What, like SteamBans? It's a plugin on the server that links all other servers with the same plugin. It bans the SteamID on every server. Then again, SteamBans is pretty rare, only seen it on a couple servers.
