Yes. I found that page. It does not list where to send in a request, that I can see. Five pages back, people were talking about how they still handle one request per person, with up to three refunds for that one request.

Cannot fucking believe it. I was playing Annie and was 10/2/5 at 15 minutes then our only AD went afk and did not return for 25 so we lost.
They all built mr and my Annie combo would still take off 50%+ of everyone of them. So retarded to lose a game when it was in the bag so easily.
